What will the cost of my physical therapy be?  Sparrow PT utilizes a SLIDING FEE SCALE for payment as a means of bridging the health care divide. If any of these fees are still preventing you from seeking care for yourself, a loved one, and/or child please let me know so we discuss the Pro Bono Clinic option.

Do you accept insurance? Sparrow PT is considered an 'out-of-network' provider with all insurance companies and does not work with Medicare. A percentage of your out-of-pocket expense will most likely be reimbursed to you directly by your insurance carrier. In order to help facilitate this Sparrow PT will provide an invoice that includes all the necessary codes and information needed by your insurance in order to reimburse you. It is recommended that you contact your insurance company in advance to learn more about your out-of-network benefits.  

How is payment collected and what is the cost?  Payment is required at time of service. Payments can be made via check or credit card via square. If you choose square please note a 3.5% processing fee will be added to your invoice.

Do I need a prescription? In the District of Columbia patients do not need a doctor's referral to receive physical therapy treatment. Note that a referral from your doctor may help minimize insurance issues and is recommended if you intend to seek reimbursement from your provider.

Where are you located? Sparrow PT provides home based PT services to the pediatric population. Adults are seen at an outpatient site at 3620 15th Street NE Washington DC 20017.

What is your cancellation Policy? When you schedule an appointment with Sparrow PT that time is reserved solely for you. When an appointment is missed without notice within a 24-hour period, there is not an opportunity to offer that time slot to someone else in need of services. Missed appointments can also interfere with a client's progress in treatment. 24 hours/1 Business day notice is required to cancel each one-hour appointment you have scheduled any late cancellations or missed appointments will be charged the full amount of a regular visit.  

How should I prepare for my first appointment and what should I wear to my sessions? Once you have scheduled an appointment, forms will be emailed to you. Please complete your new patient forms and either bring them with you or email them in advance. If you do not have access to a computer Sparrow PT will review policies over the phone with you and provide the forms in person on the first visit. Wear loose, comfortable clothing and athletic shoes.
